In the aftermath of the 1999 Institute of Medicine report, "To Err is Human," Americans were left with some chilling statistics. Medical error is the eighth leading cause of death in the United States, killing more than 98,000 people and costing over $17 billion each year. Despite this, few patients and physicians perceive medical error as a significant problem. Increased awareness and a willingness to fix our system of health care delivery, rather than ascribing individual blame, are the first steps in recovery.
